Background
The hospital discharge process was identified as an area of local concern. North Tyneside Council's Health and Wellbeing Sub-Committee were also looking at how hospital discharge procedures could be improved.

Previous LINk activity
December 2009: LINk held a hospital discharge event in December 2009 at The Oxford Centre, Longbenton, which was attended by 21 people. We heard presentations from Georgia Douglas, Team Manager of the Hospital Discharge Team at North Tyneside Council and Lorna Dace, Operational Services Manager of Acute Medicine and Critical Care at Northumbria NHS Foundation Trust. Both talked about current hospital discharge procedures and how their teams work, then answered questions from the audience. Round table discussions followed and LINk members were given the chance to discuss their own experiences and ideas for improvement. A report from this event can be downloaded here.
September 2009: LINk held a meeting between Northumbria Healthcare NHS Foundation Trust, Adult Social Care Services and several voluntary sector organisations in North Tyneside. Medical and social care staff were able to share information about their roles in the hospital discharge process, and voluntary organisations shared information on the support services they can offer patients and carers.

March 2009: LINk held a lively and well-attended event at Segedunum in Wallsend to gather people's experiences of hospital discharge (both good and bad) and suggestions for how the process could be improved. The information shared with us has gone into a report that has been given to the Health and Wellbeing Sub-Committee, and which will be used to help shape future plans and changes.
